Position Summary:Provides primary support for clients and families with addictions and persistent mental health illnesses. Hours: 40 hours per week Qualifications Required:Masters degree in relevant field5 - 7 years of prior experience working with people with addictions and mental illnessesPost secondary school education and training, including internships and other supervised practical experiences in a clinical or rehabilitation setting with persons with severe and persistent mental illnessMust be licensed or certified to work in the field, abide by a code of ethical and practice standards and be committed to a process of lifetime knowledge and skills developmentMust have skills and competencies to establish supportive trusting relationships with persons with severe and persistent mental illnesses as well as family and other members or the clients networkMust have a valid drivers license and a clean drivers record Qualifications Desired:Prior experience in any / or a combination of the following areas:Cognitive Behavior TherapyDialectical Behavior TherapyNarrative TherapyAddictions Counseling, Motivational Interviewing, Harm ReductionPsychiatric NursingVocational/Educational CounselingSpiritual CounselingMentalization Training Essential Functions and Responsibilities:Provide ongoing assessment of clients symptoms and response to treatment. Make appropriate changes to treatment plan to ensure immediate interventions are provided in response to changes in behaviors/mental statusProvide direct clinical services including individual supportive therapy to clients on an individual, group, and family basis in the office and in community settings through psycho social treatment model with client determined goalsProvide service coordination (case management) for an assigned group of clients including coordinating and monitoring the activities of the individual treatment teamsAssume primary responsibility for developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the client and the ITTEducate and support clients families and advocate for clients rights and preferencesConduct holistic and comprehensive assessments along with other team membersAct as liaison and consult with community agencies and families to maintain coordination in the treatment processProvide transportation to clients (on an ad hoc basis)With other team members, participate in on-call rotation for crisis intervention/support 7days/wk, 24hrs/dayDocument client progress to maintain a permanent record of client activity according to established methods and procedures To apply, for more information, and questions, email Annalisa Lafferty at ...@ellenhorn.com.
