Main Function:

Provide on-going training and personal assistance to participants with disabilities with the intent of promoting independence and community inclusion.

Duties:

  • Encourage and implement the choices each participant desires within the parameters of the program
  • Assist participants in making healthy choices that promote skill building and independence
  • Follow dietary guidelines as directed in habilitation plan
  • Provide training and assistance in area of ADLs with participants as designated in the individual plan (including: bathing, grooming, toileting, dressing, laundry, etc.)
  • Demonstrate sensitivity and responsiveness to participants by using person centered approach

  • Work with supervisors and co-workers to meet program needs
  • Work closely and effectively with participants circle of support
  • Develop an appropriate rapport with participants

  • Keep written and verbal information shared with team members regarding a participant confidential
  • Work with other service providers in the community in a professional manner
  • Participate in regular staff meetings and required training's

  • Provide support in the development and implementation of the individualized habilitation plan
  • Complete documentation based on program requirements following established guidelines
  • Carry out responsibilities as assigned by the Program Manager (i.e., doctors appointments, religious observances, banking, counseling sessions, monthly summaries, goal plans, etc.)
  • Utilize pro-active approach to avert crisis situations
  • Follow on-call procedures to receive instruction and report emergency situations; utilize on-call administrator if needed

  • Provide opportunity and advocacy in the area of continuing education, technology access, communication systems, and emotional well-being
  • Encourage and support each participant utilizing various modes of transportation, which will enable the participants to engage in a variety of community based activities. Provide instruction in the use of transportation
  • Provide support to participants in choosing and participating in a variety of meaningful recreational, vocational, and daily living skills activities based on choice and aimed at inclusion in the community
  • Research community resources and provide linkage to increase inclusion within the participants community
  • Foster independence and autonomy
  • 7 Other duties as assigned

    Scope of Responsibility:

    With on-going supervision, provide training, role modeling, structured routines and specialized assistance as outlined in each participants habilitation plan. As much as possible, routines will be individualized, based on the participants choice of activities and aimed at empowerment and inclusion in the community. Contact with public moderate.

    Education & Experience:

    High school diploma/GED. Experience working with people with disabilities preferred.

    Conditions of Employment:

    Must have a valid NYS driver license in accordance with the Agency Vehicle and Driving policy. Must be able to lift 75 pounds. Clean and reliable transportation adequate to meet program needs. Flexible schedule to meet program needs. Successful completion of required in-service training. Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the State of New York, Office of People with Development Disabilities, Chapter 575 of the Laws of 2004. Must participate in successful database checks through the HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specially Designated Nationals.
