Employment Plan Coordinator Jobs Login Syracuse, NY, USA Full Time Email Me Similar Jobs Email Me This Job Employment Plan Coordinator Full-Time Opening The Employment Plan Coordinator is responsible for overseeing the employment related plan of services based on person centered planning concepts that are delivered to each assigned individual. The Employment Plan Coordinator works with the person and their support team to develop and revise the habilitation goals with a focus on areas that are most important to the person, and is ultimately responsible for ensuring consistent implementation and documentation of all aspects of the individual's program plan. The Employment Plan Coordinator develops these services in compliance with all applicable state and federal regulations. Requirements Develop Waiver/SEMP Habilitation Plan and ACCES-VR Extended Services Plan annually for each individual using person centered planning, along with corresponding documentation mechanisms as necessary and review semi-annually. Attend annual and semi-annual review meetings for each individual assigned. Ensures appropriate follow up on recommendations. Works closely with the person and their support team to ensure services, health related and otherwise, are implemented appropriately. Develops monthly summaries for services each person receives. Analyze data and determine progress/ regression, etc. Revises program plan as needed. Conduct on-site observations and provide feedback to staff to ensure proper, consistent implementation of goals and plans. Completes the four-day POM Interviewer Workshop and actively complete POM interviews as assigned. Incorporates individual's POM results into their habilitation plans as appropriate Possess effective interpersonal and communication skills to handle sensitive issues with individuals and the people who support them. Maintain open communication with Medicaid Service Coordinators, families and program staff. Attend staff meetings as appropriate and provide training to staff regarding implementation of all aspects of the plan. Participation in the quality assurance process as assigned by the Employment Options Manager. Maintains individual files and record keeping procedures for quality and compliance in accordance with OPWDD as well as local, state and federal regulations including ACCES-VR. Ensures the timely completion of required assessments, monthly summaries and habilitation plans in accordance with regulatory requirements and agency policies. Ensures habilitation plans are sent to MSC's within the required regulatory time frame. Establishes and provides clear and legible documentation and compliance records required by Arc and other government agencies. Expected to conduct all agency business and complete job responsibilities in accordance with agency Code of Conduct and Corporate Compliance Plan Must be able to travel to various program locations on a regular schedule to provide ongoing oversight to the implementation of the individual's habilitation plan. Responsible for providing safe transportation as necessary or required to individuals to and from off-site locations in agency owned vehicle and/or own vehicle. Enters and maintains all information within the New York Employment Services System (NYESS) in an accurate and timely manner as required. Supports the Goals and Mission of Arc of Onondaga. Ensures that your communications and personal performance reflect the Agency's commitment to teamwork, compliance with policies and procedures and respect for participants and staff. Computer and office technology expertise that is required to fulfill job responsibilities and assignments. Attends and participates in required Arc meetings, staff development and training programs. Attends all OPWDD Mandated Employment Supports Trainings. Provides other job-related support and services as required or assigned by management Education & Experience Requirements Bachelor's Degree in Human Service field and one year experience of working with individuals with developmental disabilities. A valid NYS driver's license and a driving record acceptable to the Agency's insurance carrier.
