We recently underwent a radical transformation, and now it's time to grow!This is a full-time, direct hire, permanent, M-F / 8-5 opportunity.If working with highly competent, thoughtful professionals who value good ideas and forward thinking Clinical leaders is something that sounds motivating to you, this might be the right opportunity!This position is responsible for leading and implementing programs and projects of the department related to UM inpatient program management, & authorizations, resource allocation, and reporting related to defined delegation requirements of our health plans, state and national standards maintaining Health Plan and regulatory compliance standards.Under the direction and supervision of the Executive of Clinical Services, the Manager of IP Services is responsible for managing and supervising the daily activities of the clinical and administrative staff performing IP Clinical Care Management activities including turn-around times and compliance standards.Assists in the operations of Care Management functions including planning, monitoring, and implementation of care for the patient, in conjunction with the physicians and any other medically needed ancillary services.Supervises the evaluations of patients’ treatment plan as needed.Directly interacts with a variety of hospital systems, the medical staff, health plans, Performance Improvement, and Physician Services.Responsible for the supervision of staff members.Responsible for managing care coordination services including identification of post acute level of care continuity of outpatient services ordered at discharge from acute facilities.This may include emergency department utilization, acute rehabilitation, skilled nursing facilities and appointments at physician offices.Continuity of care coordination may be done through handoffs to internal and community partners.This position develops and monitors processes that support quality of care throughout the continuum of care.Job Requirements:California RN / Registered Nurse License2+ years of Management experience overseeing RN Case Manager's5+ years of experience of inpatient case management experience within a Health Plan, Medical Group, or HospitalPlease Note: Unfortunately, Home Health, Hospice, or SNF experience will not be considered towards the minimum requirements.SDL2017
