Job Summary:

Facilitate Family Team conferences throughout Preventive Services and represent JCCA at client conferences with ACS.

Essential Generic Job Functions:

  • Facilitate Family Team Conferences and Service Termination Conferences as well as represent JCCA at Elevated Risk Conferences.
  • Ensure that conferences involve children, families, and community resources.
  • Identify and fully evaluate and address any and all safety and risk concerns and ensure that they are addressed in the conference.
  • Ensure that all recommendations to JCCA case planners and other service providers are communicated and an action plan is formulated.
  • Complete timely documentation in Connections and any other agency data base required including but not limited to the FTC Summary, Action Plan, Approvals, Progress notes, PROMIS etc.
  • Facilitate at minimum two to three conferences per day. Attend inter/intra agency meetings to share information, develop policies and procedures.
  • Ensure that supervisors and teams are well prepared for FTC conferences.
  • Provide feedback to supervisors and staff regarding their participation and involvement during FTC conferences.
  • Participate in monthly supervisors meeting and monthly staff meetings.
  • Provide supervisory coverage for the program, as needed, including evenings.
  • Participate in monthly supervisory forum.
  • Community outreach.
  • Other related duties as assigned by supervisor.
  • Contacts: All JCCA personnel, external agencies and vendors.

    Knowledge, Education and Skills Required:

  • MSW with minimum of one year of experience and a strong evaluation.
  • Strong leadership and group skills to work with intense family challenges and situations.
  • Ability to work in a strength based approach
  • Knowledge and skill with FTC group conferencing processes (FTC Trained).
  • Demonstrated ability to lead diverse groups toward resolution of difference.
  • Good organizational, written and verbal communication skills.
  • Must be willing to work a flexible schedule that includes evenings.
  • Demonstrated skills in mediation processes.
  • Knowledge, Education and Skills Preferred:

  • Bilingual (English/Spanish) a plus.
  • Supervisory experience a plus.

  • Physical Requirements:

  • Approximately 70% sitting, 30% standing
  • General mobility in andout of and throughout multi-floor office buildings
  • Regular daily use of PC keyboard, telephone, and other general office equipment
  • Ability to travel throughout NYS to various worksite/vendor locations
