Elmhurst Hospital Center (EHC) is the major tertiary care provider in the borough of Queens. The hospital is comprised of 545 beds and is a Level I Trauma Center, an Emergency Heart Care Station and a 911 Receiving Hospital. It is the premiere health care organization for key areas such as Surgery, Cardiology, Women's Health, Pediatrics, Rehabilitation Medicine, Renal and Mental Health Services. Job Description 1. Provides psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs; integrates nursing and medical concepts in planning care for medically complicated psychiatric patients. 2. Participates in the creation of an individualized treatment plan, after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment and other pertinent information; implements plans for patient care utilizing protocols approved by the Medical Director or appropriate designee and/or treatment plans reflecting the current standards of care. 3. Evaluates patients for the need for a higher level of care, including hospitalization. If a patient is hospitalized, maintains contact with the hospital psychiatric staff, and multidisciplinary health care team, to ensure continuity of patient care. 4. Provides health education and counseling to patients and families regarding health status, treatment and progress, medication management, disease prevention, and health promotion, and provides special consultation regarding issues related to medications and treatment adherence, and aftercare services, using advanced psychotherapeutic techniques, including in group settings, if needed; documents all activities in an electronic medical record in a timely, concise, and clear manner. 5. Provides consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment, and/or the need for psychiatric medication. 6. Appropriately prescribes medication regimens for the patient, utilizing an outcome-oriented approach to assess patient?s response to prescribed regimens. 7. Participates in team meetings with the treatment staff, on a daily basis, if working on a mental observation unit and on a weekly basis, if working in one of the clinics. 8. Engages in weekly clinical supervision with the Senior or Supervising Psychiatrist; provides clinical supervision for Psychiatric Nurse Practitioner trainees as needed, including appropriate direct supervision and documentation of evaluation and feedback. 9. Performs other duties, as assigned by the Senior Psychiatrist or Mental Health Manager for the facility, Medical Director or Chief of Service for Psychiatry, or their respective designees Responsibilities 1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Education Department (NYSED). 2. Current certification to practice as a Psychiatric Nurse Practitioner issued by the NYSED. 3. Current Basic Cardiac Life Support (BCLS). 4. Holds and maintains prescriptive privileges. 5. If qualified under the Nurse Practitioners Modernization Act (the Act), may choose to practice and have collaborative relationship(s) with one or more physician(s) qualified to collaborate in the Nurse Practitioner?s specialty area(s), as allowed by the Act.
